What is Connecting Care?

Connecting Care is a digital care record system for sharing information in Bristol, North Somerset and South Gloucestershire.  It allows instant, secure access to your health and social care records for the professionals involved in your care. 

Relevant information from your digital records is shared with people who look after you.  This gives them up-to-date information making your care safer and more efficient.

Connecting Care means safer and faster treatment, reduced duplication of tests and a much more joined-up approach to the care you receive

Is my information secure?

More than 10,000 health and social care professionals in Bristol, North Somerset and South Gloucestershire could have access to your records thanks to Connecting Care, so it is vital that the right controls are in place to protect your personal information. Your records are stored in a highly secure professional data centre in England and only those directly involved in your care can access information about you.

Seventeen health and social care organisations are behind Connecting Care. Together, we care for nearly one million people in Bristol, North Somerset and South Gloucestershire.

This website tells you everything you need to know about Connecting Care. You do not need to do anything further if you are happy for your information to be shared locally with the people who look after you.

Only patients who are registered with a GP practice in Bristol, North Somerset or South Gloucestershire will have their GP information included in Connecting Care.

Yes. You have the right in law to ask for a copy of any information organisations hold about you. This includes copies of paper, digital and hybrid patient health records. 

If you wish to access information held about you on your health and care records, please contact the organisation(s) providing your care.  The information displayed in Connecting Care is only a partial record of what is held about you by the Connecting Care partner organisations.  If you would like to request a copy of the of your information held in Connecting Care, you should send your request in writing to: Freepost, CONNECTING CARE SWCSU or email bnssg.pals@nhs.net
