Connecting Care makes it possible to share information instantly to support your care.  It creates an ‘integrated digital care record’ for people who are registered with a GP working in Bristol, North Somerset or South Gloucestershire.

You may also have heard about a national project known as Summary Care Records. This is very different to Connecting Care.


Summary Care Record

The Summary Care Record (SCR) is a way for the NHS to allow some of your information to be shared nationally between clinicians to support urgent care. Unlike Connecting Care, a Summary Care Record does not provide a complete picture of your health and social care. NHS Choices has more information about Summary Care Record


Local Health Care Records Exemplars

You may also have heard about a national project known as Local Health Care Records Exemplars (LHCRE).

This is different to Connecting Care.

As part of the evolution of digital information sharing in health and social care, there is a national movement towards sharing at a more regional level, for example, across the south-west. 

For more details on LHCREs visit the NHS England website.

This initiative is still in the early stages, and there is currently no LHCRE system in the south-west.
